We ask potential users to provide the following personal information during registration:

Gender and age are needed to establish a baseline for the various health tools inside the site. An e-mail address is gathered as your unique identifier. There is no requirement that your user name be your real name.
StayWell Solutions Online does not share e-mail addresses with any third party, including the licensee.
We automatically track and collect the following information when you visit StayWell Solutions Online:
Updating personal information
Registered users may update their personally identifiable information and personal health information at any time. We recommend that users who take the Health Risk Assessment review that information at least annually.
Use of cookies
We use passive tracking mechanisms called cookies to track the use of different features and areas of StayWell Solutions Online. These cookies are stored on a user's computer hard drive.
A cookie is also used to identify you after logging in. The cookie that identifies you to our site is not permanently stored on your computer’s hard drive. It is cleared when you exit your web browser. This session cookie becomes invalid if you do not navigate within the site for 20 minutes or if you log out. This helps prevent other users from accessing your account information.
Deleting stored cookies on your computer will not affect your use of the site, but you will not be able to access personal information you have entered. If “session” cookies are disabled, you will not be able to log in.
Data gathered by licensees
StayWell Solutions Online is only available through licensees. When the licensee gathers more demographic data than required by StayWell Solutions Online that collection will be covered in the licensee's privacy statement. In some cases, StayWell Solutions Online shares a single login with the licensee.
In those cases, the information passed to StayWell Solutions Online is limited to: username, password, password reminder, screen name, e-mail address, age, and gender.)

Health Mirror health risk assessment
To the extent that the registered user opts to provide information about his or her medical history, diseases, conditions, behaviors, and goals when taking the Health Mirror Health Risk Assessment, that information is identified with the StayWell Solutions Online registered user. This personal health information is stored on StayWell Solutions Online servers, not on a licensee's server.
Registered users can opt not to provide requested personal health information when taking the Health Risk Assessment. The user should understand that withholding that information will affect the ability of the tool to provide relevant information.
Requesting e-mails
Various tools in the post-login sections of StayWell Solutions Online offer the option of receiving e-mails. In no case are e-mails broadcast to users without their permission. Users opt to receive any e-mails and users can cancel e-mail delivery at any time by updating their profiles.
StayWell Solutions Online does not share e-mail addresses with any third party.
Calculators, quizzes, and risk assessments
Except for the Health Mirror Health Risk Assessment, the calculators, quizzes, and risk assessments available on StayWell Solutions Online do not collect any personally identifiable information, nor do they track who is using the tool.
Divulging personal information
StayWell Solutions Online never shares personally identifiable information or personal health information with any third party, including the licensee, except under court order.
Aggregate reporting
Licensees are able to review on-demand reports on site traffic. These reports focus on total visitor sessions, most frequently viewed pages and referring sites. No personally identifiable information is collected for these reports.
Maintenance of data
The personal data that users submit when using various StayWell Solutions Online applications is maintained on servers owned and operated by StayWell Interactive. Backup to tape are made weekly and data remains in a secure off-site storage location for seven years. After such period of time, the backup tapes are destroyed. However, active data may remain on the server indefinitely and would be erased only in the event of a hardware failure, policy change or discontinuance of service by StayWell Solutions and/or StayWell Interactive.
Registering complaints and deleting accounts
If you wish to register a formal complaint about an alleged violation of the HIPAA Privacy rules, that correspondence should be addressed to:

Registered users also may cancel their accounts and have their personal information deleted from active databases by sending a signed statement containing name, date of birth, postal address and e-mail address to Mr. Rademaekers at the above address.
StayWell Solutions Online commits to deleting your personal information from all active versions of the service within 30 days of receipt of the letter. The former registrant will receive an e-mail confirmation that their personal data has been deleted. However, former registered users should understand that their personal information will reside on archived backup media not connected to the Internet--for seven years.
Links to other sites
Some StayWell Solutions Online content contains links to other Web sites. While we carefully select which links to include, we can't control nor guarantee the ongoing quality of the information. These sites may have different privacy and commerce policies, which visitors should review before providing any nformation.
Changes to the privacy policy
StayWell Solutions Online reserves the right to change the security policy from time to time. The date of the last modification to the policy appears at the bottom of this document.
